Testing Reservation Form

For students needing to request a testing voucher or request a time for an online test, please complete and submit the Voucher Request Form.
 

Step 1

Complete the Voucher Request form below. Be sure to select for which exam you need the voucher.

If taking the TSIA2, the Pre-Assessment Activity (PAA) is required and must be completed before a voucher can be created.

Step 2

Check your email for voucher code and directions on how to register for your test. Voucher requests can take 1-3 working days to create.

Step 3

Upon receipt of voucher, contact Meazure Learning for an appointment. Details will be in the email received from Meazure Learning. 

** Please note, this voucher is ONLY for those who are wanting to take the test ONLINE.  The voucher will not work at the Midland College Testing Center. **
