You have chosen to request a unique identifier assigned to you while at Midland College. Please note that you have the right to an alternate identifier, but please note the consequences of not providing the social security number:
You will be assigned a unique 9-digit alternate identification number.
You must remember and supply the alternate unique identification number that is assigned to you in order to receive services/transcripts from the college.
The unique identification number will not be transferable to any other higher education institutions.
You will not be eligible to receive financial aid without providing your social security number.
You will not be able to receive a 1098-T for tax purposes without the Social Security Number.
Future employers will not be able to confirm that a certificate or degree has been earned at Midland College without the unique identification number that you supply them. If they try to use your actual Social Security Number, we will not have that information in our database.
By leaving this option selected I agree that I have read and understand the outcome of not providing my social security number and I request a unique student identifier to be assigned to my records.
