Transcript Request

Please complete the form below to request a transcript. There is no charge for transcripts unless "Expedited Overnight FedEx Service" is requested.

Transcripts are generally sent within 2 - 3 business days upon receipt of request excluding weekends and holidays. All fields are required; incomplete requests cannot be processed. Transcript requests for students who have holds will not be processed until the hold(s) have been removed by the appropriate office on campus.  Students will be notified if they have a hold which blocks processing.

Because of the Family Educational Rights and Privacy Act (FERPA) regulations, Midland College is not allowed to mail transcripts anywhere but to the student’s home address or an institution of higher learning without authenticating identity. If you need a transcript sent directly to an application service, business, insurance company, or licensing agency, etc., you must submit the online request AND upload the Transcript Request and Authorization form that has your signature and date. Students who cannot complete and upload the Transcript Request and Authorization form may request to have the transcript sent to you, and you can forward it to the other types of institutions or agencies. NOTE: We do not email transcripts.

Midland College makes every effort to protect a student’s privacy. The information submitted will be checked against the student’s records for accuracy. Midland College retains the right to deny the online method of transcript request for any reason it deems appropriate. If the online request is denied, a transcript request and authorization form that has the student’s signature and date will be required before a transcript will be sent.

*1) I understand that online transcript requests that do not include the authorization form will result in the transcript being sent to the student’s address or to a college or university only.
*2) I understand that I must submit the Transcript Request and Authorization form that is completed, signed, dated, and uploaded to Midland College in order for my transcript request to be processed for a third-party such as a centralized application service, licensing agency, business, insurance agency, or parent.
*3) I understand that transcripts will not be sent if there are holds on my student account.
*4) I understand that, should I choose to use it, “Expedited Overnight FedEx Service” requires receipt of an authorized request and online payment authorization by 3:00 p.m. in order to be processed same day. Requests or payments received after 3 p.m. will be processed the next day.
