Forms You Will Need
Selecting which form the VA requires is determined in part by which VA chapter applies to you. The most commonly utilized chapters are as follows:
Chapter 30 . . . . . . Montgomery GI Bill (most veterans)
Chapter 31 . . . . . . Vocational Rehabilitation (disabled veterans)
Chapter 35 . . . . . . Dependents (of disabled veterans or veterans killed in service)
Chapter 1606 . . . . Guards or Reservists
Chapter 1607 . . . . Guards or Reservists (active duty since 9/11/01
All students using benefits must complete the Request for Certification form every semester .
If you have used your VA benefits before at another college and are transferring to MC, you will need the following forms:
Chapters 30, 1606, 1607: VA Form 22-1995, DD-214 Member 4, MC VA Student Data Sheet
Chapter 31: VA Form 2P-1905 (from VR&E Counselor)
Chapter 35: VA Form 22-5495, MC VA Student Data Sheet
If you are using your VA education benefits for the first time , you will need the following forms:
Chapter 30: VA Form 22-1990, DD-214 Member 4, MC VA Student Data Sheet
Chapter 31: Contact VR&E counselor Kihm Jones to receive VA Form 28-1905. Her contact
