All students using benefits must complete the Request for Certification Form every semester.

 

Selecting which form the VA requires is determined in part by which VA chapter applies to you. The most commonly utilized chapters are as follows:

 

• Chapter 30 - Montgomery GI Bill (active duty service)
Chapter 31 - Vocational Rehabilitation (disabled veterans)
Chapter 33 - Post-9/11 GI Bill (active duty since 9/11/01)
Chapter 35 - Dependents (of disabled veterans or veterans killed in service)
Chapter 1606 - Guard or Reservists
Chapter 1607 - Guard or Reservists (active duty since 9/11/01)

• VRAP - Veterans Retraining Assistance Program

 

If you intend to establish residence in Texas and reside in Texas while enrolled at Midland College, please complete & submit this form to the MC Admissions Office:  Intent to Establish Residence in Texas for Veterans and their Family Members

 

 If you have used your VA benefits before at another college and are transferring to MC, you will need the following forms:

 

Chapters 30, 33, 1606, 1607 - Certificate of Eligibility, DD-214 Member 4, Military Transcript, transcripts

    from all institutions attended, and MC VA Student Data Sheet
Chapter 31 - VA Form 28-1905 (from VR&E Counselor), DD-214 Member 4, Military Transcript, transcripts

    from all institutions attended, and MC VA Student Data Sheet
Chapter 35 - Certificate of Eligibility, DD-214 Member 4, Military Transcript, transcripts

    from all institutions attended, and MC VA Student Data Sheet

 

 If you are using your VA education benefits for the first time, you will need the following forms:

 

Chapter 30 - Certificate of Eligibility, DD-214 Member 4, Military Transcript, transcripts from

   all institutions attended, and MC VA Student Data Sheet 
Chapter 31 - Contact VR&E counselors, Melvin Rand or John Kraft, to receive VA Form 28-1905. 

   You will also need DD-214 Member 4, Military Transcript, transcripts from all institutions attended,

    and MC VA Student Data Sheet
